Abstract: A relatively mild nonoliguric acute renal failure developed early in the course of daily rifampin treatment in a 54-year-old man who was receiving concomitant steroid therapy. The renal lesion was an acute interstitial nephritis characterized by tubular dysfunction:inability to concentrate urine, increase in fractional excretion of sodium and uric acid, and glycosuria with normal blood glucose values. All abnormalities resolved promptly upon withdrawal of rifampin while steroid therapy was gradually reduced. The documents the fact that rifampin-induced nephropathy can occur in the course of a continuous daily regimen and may be favorably influenced by steroid administration.(AU). Abstract: A massive dose of 7.5 g of 4,4'-diamino, diphenyl sulphone (dapsone) taken as a suicide attempt in a patient on long-term therapy for tuberculoid leprosy resulted in permanent bilateral retinal necrosis, previously unreported side effect of this drug. The patient developed a severe haemolytic anaemia, methaemoglobinaemia, and acute renal failure requiring peritoneal dialysis. It is proposed that the retinal damage was due to a combination of severe hypoxaemia and the physical effects of red cell fragmentation producing vascular occlusion in the macular and perimacular region, with consequent ischaemic necrosis...(AU). Abstract: Sete pacientes consecutivos, de um grupo de 283 portadores de Hanseníase em tratamento com esquema terapêutico multidroga (MDT), foram referidos ao Hospital Evangélico de Londrina e ao Hospital Universitário Regional Norte do Paraná, no período de 1989 a Novembro de 1991, devido a insuficiência renal aguda (I.R.A.). Nestes sete pacientes ocorreram oito episódios de IRA, acompanhados de epigastralgia (8/8), febre (6/8), vômitos (5/8), anúria (4/8), urina escura (4/8), dores ósseas acompanhadas de mialgias (5/8) e icterícia (3/8). Quatro pacientes necessitaram tratamento dialítico, e três foram submetidos à biópsia renal que demonstrou Nefrite Rúbulo Intersticial Aguda. Os autores atribuem estas manifestaçöes ao uso intermitente da rifampicina e sugerem cuidados a serem observados quando se usa esquema multidroga (MDT). (AU).
